You understand that specimen collection involves certain inherent risks, including but not limited to bruising, fainting, infection at the venipuncture site, excessive bleeding, or hematoma formation.

12. Indemnification

You agree to indemnify, defend, and hold harmless the Provider, its affiliates, officers, directors, employees, and agents from any claims, liabilities, damages, judgments, awards, losses, costs, or expenses (including reasonable attorneys' fees) arising out of or relating to your use of the laboratory testing services, your violation of these terms, or your violation of any rights of a third party.

13. Governing Law and Dispute Resolution

This Acknowledgement shall be governed by and construed in accordance with the laws of the state in which the Provider is organized, without regard to its conflict of law provisions. Any disputes arising under or in connection with this Acknowledgement shall be resolved through binding arbitration administered in accordance with applicable rules, and judgment on the award may be entered in any court having jurisdiction.

14. Severability

If any provision of this Acknowledgement is found to be unenforceable or invalid under applicable law, such unenforceability or invalidity shall not render this Acknowledgement unenforceable or invalid as a whole.
