Should you have any issues related to your lab draw, ordering process, or results, you must contact the ordering laboratory directly: DHA Labs.

By proceeding and signing this acknowledgment, you understand and agree to the following:

  • I, Free Dominguez CNC FDNP, am not involved in the processing, fulfillment, or handling of your bloodwork order beyond providing access to the third-party link.

  • I do not have access to your personal information (including date of birth, address, payment information, or lab results). Any contact details, health information, or order information you provide is submitted directly to the third-party laboratory.

  • I do not receive, view, or have access to your laboratory results. All results are provided directly to you by the laboratory service.

  • I do not provide medical advice, diagnosis, or treatment.

All laboratory testing and associated materials are provided strictly for informational and educational purposes only.

Any results obtained should be reviewed with your licensed healthcare provider. You should consult your physician or qualified medical professional before making any medical decisions or starting any treatment based on your results.
